Students aged 14-17 are required to submit a Work Permit to their employer. The application for a work permit for ages 14-17 can be found here.

Please note: physicals must be within a year of the application. You can also have your doctor fill out the attached Physical Fitness Certification (signed and stamped by your doctor) in lieu of an updated physical.

Once you complete this form, you can pick up your working card from the guidance office. (The student must be present to sign the Working Card before it can be validated)
